This form sends a request to the practice you choose. It is not a live booking system: nothing is confirmed until a colleague has looked at your prescription and called or written back.

What happens after you send it
Requests are read during opening hours at the practice you selected. Reception checks the prescription, offers you a time and — if the practice is full — checks the other five before answering. You will hear back within one working day. If you need something today, please call.
The waiting list
If nothing is free, ask to be put on the group waiting list. It runs across all six practices, and Viktor Marek calls when a slot opens — including at a practice you did not ask for. Roughly one in four appointments each week is filled that way.
